Our care service options are designed to best accommodate the level of care services desired within the cost of care budget for each family.

3 Care Service Options:

Silver | Gold | Platinum

All options support hourly and live-in care.

Silver Care

Our clients who elect the Silver Level of Care are those clients whom hire their own caregivers (often referred by us) and would like some assistance during the first 90 days with establishing best practices for care coordination and overall care management.

Silver Clients are provided:

  • Unlimited Use of the Family Learning Center for Caregiver Training
  • The Sesen Elder Care “Care Plan Organizer” with all of the Sesen Elder Care forms and care management tools
  • Training on the Sesen Elder Care best practices and home care procedures
  • Tools for caregiver scheduling and caregiver payment
  • Complete access to the Sesen Elder Care team of elder care professionals

By using our care management services, family education and a network of trusted care providers, we provide our Silver level clients the opportunity to experience a substantial improvement in the quality of in-home care while reducing their overall cost of care.


Gold Care

Our clients who elect the Gold Level of Care utilize the Sesen Caregiver staff to provide in-home care. Our employees are some of the best, experienced providers of elder care in the Bay Area, and are at the very core of our ability to provide exceptional care. Gold Level clients also utilize and experience the benefits of our care coordination and care management services, our family education services and referrals to our team of trusted care providers.

Gold Clients are provided:

  • Unlimited Use of the Family Learning Center
  • The Sesen Elder Care “Care Plan Organizer” with all of the Sesen Elder Care forms and care management tools
  • On-going access to the Sesen Elder Care team of elder care professionals
  • On-going care coordination and care management, which includes:
The Family Care Portal
  • Caregiver Calendar
  • Appointment Calendar
  • Daily care activity charts
In-home Care Reviews
  • Weekly care plan review
  • In-home family care conferences


Platinum Care

Similar to our Gold Level of Care clients, our Platinum Level clients utilize the Sesen caregiver staff to provide in-home care. However, due to geographic distance and/or time constraints, our Platinum clients elect to take advantage of the full breadth of the Sesen care management and care coordination services. Our Platinum level clients experience the same high quality of in-home care coupled with on-going elder health care advocacy and advice.

Platinum Level of Care improves overall elder care advocacy and health follow ups while requiring the least amount of family time consumed to support elder care – and allowing more time for families to share the important life moments.

Platinum Clients are provided:

  • Unlimited Use of the Family Learning Center
  • The Sesen Elder Care “Care Plan Organizer” with all of the Sesen Elder Care forms and care management tools
  • On-going access to the Sesen Elder Care team of elder care professionals
  • On-going care coordination and care management, which includes:
The Family Care Portal
  • Caregiver Calendar
  • Appointment Calendar
  • Daily care activity charts
In-home Care Reviews
  • Weekly care plan review
  • In-home family care conferences
Coordination and Management of:
  • Doctor Appointments (arrange and attend)
  • Prescription Medication Refills
  • Home Safety Modifications
  • Provider referrals (medical specialists, therapeutic specialists, family counseling, relocation services)
  • Social and Community activities
